Job Description
The position hour range from 8:30am to 5:00 pm, Monday through Friday. Hours vary 30 - 40 hours a week, salary $12 to $14 per hour.
The Patient Navigator provides advocacy, referrals, information and connects patients with specialty care services not currently provide on-site at Christ Clinic.
Duties include but are not limited to, advocating for patient’s needs, setting up appointments for specialty care, assisting patients with Prescription Assistance Program applications to receive needed name brand medications.
Qualifications:
Associated degree (preferred), Bilingual, Spanish/English (required). Social Worker, Community Health Worker, Pharm Tech or Medical Assistant’s encouraged to apply. Non-Profit experience is a plus. Organized and detail oriented, excellent communication skills, ability to work alone without direction, willingness to learn and grow with the changing requirements of the job.
